How to Request a Transcript

Past Graduates


This process is only for past students: 

Transcript requests are only available online for graduates 2001-present. If you graduated prior to 2001, please contact Middletown City Schools at (513) 423-0781.

Please email the following required information to Mrs. Wehry at in order to process your transcript request:


Current Students


If you need a transcript for your college application:

  • You must request your transcript order online through Naviance for free.
  • Use this link to request.
  • Log in using your username (FirstnameLastname - no spaces) and reset your password by clicking 'forgot password.'


If you need your transcript for personal use or for a scholarship application:


Education Verification


Mail check or money order ($5) and the signed release (this can be electronic) to 220 Yankee Rd. Monroe, OH 45050. We do not accept faxed or email requests. 

     *You must include your graduation year in order to process.
