Monroe Local Schools has a clinic in each building. The school clinic provides services such as, but not limited to:
- Maintaining immunization status.
- Providing daily care to ill or injured students.
- Emergency care and treatments for students with health impairments.
- Providing vision screenings to all students in preschool, kindergarten, first grade, third grade, fifth grade, seventh grade, ninth grade, 11th grade, and all new students to the district.
- Providing hearing screenings to all students in preschool, kindergarten, first grade, third grade, fifth grade, ninth grade, 11th grade, and all new students to the district.
- Providing routine head lice checks.
- Promoting health education for staff and students.

Medications
The school medication permit must be completed if your child will need to take over-the-counter (OTC) medication such as Tylenol, Advil, cough drops, etc.) or prescription medication during school hours. Both you and your child’s doctor or licensed prescriber must complete sections of this form. This form must be completed each year for prescription and nonprescription/OTC medications by both the parent and physician.
- All medication must be received in the school clinic directly from a parent/guardian; and controlled substances, such as Ritalin or Adderall, must be counted at school when designated staff receive the medication. The parent/guardian should remain present to verify the pill count.
- All medications must be in the original container properly labeled with the student’s name. Please never send medications to school in envelopes, plastic baggies, backpacks, or in your child’s pockets. These medications will not be given.
- A separate school medication permit must be completed for each medication that the student will be taking, signed by both the parent and the physician.
- All medication must be picked up by the Friday after the last day of school. Any leftover medications will be properly disposed of.
The medication will be administered, as ordered, by a staff member trained to administer medications. Clinic staff will not administer medication outside the parameters listed on the completed medication permit. If the school has a delayed start and arrival is after the normal time for the student to receive their medication in the clinic, the parent/guardian will need to make arrangements to have the medication administered prior to going to school.

- The parent/guardian must complete the first box. Leave the second box blank unless you and your child’s doctor authorize your child to carry and self-administer either an asthma inhaler or an epinephrine auto injector.
- The child’s doctor or licensed prescriber must complete the third box.

Please note: All of the following conditions must be met or your child’s asthma inhaler or epinephrine auto injector will be stored in the school clinic and administered by trained staff:
- The parent/guardian must complete the first box on the school medication permit.
- The parent/guardian must complete the second box on the school medication permit, giving parental/guardian permission for the child to carry and self-administer the medication.
- The child’s doctor or licensed prescriber must authorize the child to carry and self-administer an epinephrine auto injector by “checking” the statement in the third box that states: _____________AUTHORIZATION FOR CHILD TO CARRY/SELF ADMINISTER AN ASTHMA INHALER OR EPINEPHRINE AUTO INJECTOR AS PRESCRIBED.
- A second epinephrine auto injector must be provided to school and stored in the school clinic if your child is authorized to carry/self-administer an epinephrine auto injector. Please call the clinic if you have any questions regarding your child’s health.
Vaccine Requirements
Ohio law requires that students have certain immunizations in order to attend school. Immunization against diphtheria, pertussis, tetanus, poliomyelitis, rubeola, rubella, and mumps, hepatitis B, varicella, and haemophilus influenza type b is required for each student unless the parent(s) files an objection. All kindergarten students are required to receive hepatitis B and varicella (chicken pox) immunizations. The board also requires tuberculosis examinations in compliance with law.
Students eligible for kindergarten and students new to the district must present written evidence of similar immunizations, or written evidence to indicate that they are in the process of receiving immunizations, to be completed no later than the day of entrance. Students entering the seventh grade must present evidence of an MMR booster.
Students failing to complete immunizations within fourteen (14) days after entering are not permitted to return to school.

Seventh Grade
The Ohio Department of Health requires all students to show proof they have received a Tdap vaccine (tetanus, diphtheria, acellular pertussis vaccine) and MCV4 vaccine (Meningococcal) before starting seventh grade. Please provide your child’s Tdap and MCV4 immunization records to the school clinic prior to the first day of seventh grade. Students not in compliance with immunization laws must be excluded from school according to the guidelines of the Ohio Department of Health.
Tdap and MCV4 vaccines may be administered by your child’s doctor, urgent care offices, Kroger Little Clinics or local health departments. To schedule an immunization appointment at the Butler County Health Department, please call (513) 887-5253.

12th Grade
The Ohio Department of Health requires all students to show proof they have received two doses of the MCV4 vaccine (Meningococcal) before starting 12th grade (only one dose is required if received on or after a student's 16th birthday). Please provide your child’s MCV4 immunization records to the school clinic prior to the first day of 12th grade. Students not in compliance with the immunization laws must be excluded from school according to the guidelines of the Ohio Department of Health.
MCV4 vaccines may be administered by your child's doctor, urgent care offices, Kroger Little Clinics or local health departments. To schedule an immunization appointment at the Butler County Health Department, please call (513) 887-5253.

Butler County General Health District Vaccine Clinics
Vaccination throughout childhood is essential because it helps provide immunity before children are exposed to potentially life-threatening diseases.
Vaccines for children are available for those 6 months - 18 years of age and receive health insurance from Medicaid or self-pay. No private insurance is accepted.
